Try buying a Flash Drive (Pen Drive) with a write protection switch. It is located on the side of a Flash Drive. Another alternative is, Not to insert it into infected systems at all!

Keep your Antivirus updated regularly incase you cannot avoid your bad habbit of inserting it in unknown or virus prone systems.

I dont know of a real-time scanner. Keep scanning regularly with ClamWin portable(on your pendrive) or use Stinger.
