How can i fix my Vauxhall astravan clutch ?

The clutch in my W reg Astravan 1.7dti is playing up. When i lift my foot off the clutch sometimes it works ok other times when you dip the clutch to take it out of gear the clutch is not disengaging & the van carries on going.I have to brake & stall it to stop.
