how can i fix my ford transit lwb?

my back breaks seem if the are stuck on when i sit on a hill it doesnt roll but my hand brake is not working right and when the problem happens i pull my hand brake up and down and it seems two release the back wheels and it goes for so long then it happens again

it depends what model you have,discs?drums? information you have failed to give,would have a look for seized cables,this is just a guess.
