loose rocker arm studs?

I have a chevy 1996 truck,with v-8 motor. Recently the stud on one head came up and caused the rocker arm to come off. I replaced it with a new stud that was tapped into the head,now the other side has one loose. It only has 88K miles and was well taken care of.Why is this happening and should I replace all the studs?

they all went in at the same time so better to replace all of them fit Loctite to the threads of each stud and torgue them to the correct tightness and fit new rocker gasket too
