Dimmer switch has to be turned on and off several times before lights will come on at all. Faulty connection or do we need to replace the dimmer switch?

Take your pick, Certainly don't use the lights until its fixed. Look to the back of the switch and see if there are loose wires to the switch, pull the fuse first but if not i'd look at replacing the switch. I don't like electrics that kinda work as there could be an overheating problem and thats dangerous.
