water keeps leeking from between cistern and toilet?

when flushing toilet the water leeks between cistern and toilet what have i done wrong and how can i fix it

When fixing syphon to cistern you should have rubber(Black) washer between syphon and inside botton of cistern and than tighten with plate and back nut and should not leak.
