how can i fix Swift flow boiler glow worm?

when its on the overflow pipe outside is continuoisly running, and pressure is always between 3 and 4.

Turn off the filling loop, if this does not fix it Heat Exchanger will have failed .. Best to Repalce Boiler if HE has failed due to age of boiler etc etc
