lukewarm domestic hot water?

glow-worm 30xi my domestic hot water only reaches hand hot when the setting is on max . domestic hot water pipe is 15mm and is connected to old 22mm domestic hot water pipe running about 18 metres in length in the loft to the bathroom can this be the problem

The 30xi should be set at max for winter months - 82C. If you have a hot water tank, is the stat on the tank set to 60?

pipe should be 15mm from the boiler to the bath, the last foot or so should be 22mm any more than this and the boiler will struggle to heat the 22mm pipe.

I doubt the pipes will be the problem. Are they lagged? (May help slightly) Have you always had the problem?
