tepid hot water?

linea 24, central htg ok, hot water from tap initially hot but then cools off and stays that way.
Engineer has changed primary thermister, no difference, he then suggests heat exchanger?
any advice welcomed.. thanks

blocked heat exchanger would be a possibility if it starts hot then goes cooler
