check colour of water in condensate trap

if its CLEAR then there must be a leak on the system

if its RUSTY BROWN then very bad news
MAIN HEAT EXCHANGER IS LEAKING INTERNALLY
no other option than to replace HEX

may be pressure relief valve is faulty did you notice any water going out side from over flow pipe from boiler ,change valve
