SD flash card died?

I have a 256 mb sd flash card (Microsim ).It is fairly new.,with a five year warranty. It has stopped working,does anyone know what to do or where to send it for replacement?

try the flash doctor program to rest the card to factory defult and it will work again u must have acard reader support this program.
