running toilet?

My toilet is constantly running! Ive tried tightening, and loosening, all the various screws inside the cistern but to no avail - any ideas?

It also takes takes 2 flushes to get rid of toilet paper!

loks like the syphon has gone ,this will need replacing if it is a close coupled toilet then you will have to take the cistern off the toilet to undo the large nut which holds the syphon into it
