Whats the problem with my 1998 ford mondeo?

I have a diesel 1998 ford mondeo td. When i pulll away the revs red line of their own accord and cause a lots of black smoke. it only does this when i pull away.

once started it runs for about 5 mins then the fans switch on run for about 1min then cut out the engine runs badly then stalls the expantion tank bubbles , there is aslo white smoke
